Access to Social Services for Stroke Survivors Living in a Low Income Situations : Implementation of an Intervention

This project is funded by the Social Sciences and Humanities Research Council of Canada. The research team is comprised of Katrine Sauvé-Schenk, Jacinthe Savard, Sébastien Savard, Cathy Pacifici and François Durand, and our research assistant Sandra Houle.

Following the systematic review and usual care research projects, this pilot study will use a collaborative approach to plan, test and implement an inter-professional intervention in the acute care hospital setting. The purpose of the intervention is to support low-income stroke survivors to access social services and community resources prior to discharge.
